A: There are no rules governing this, to my knowledge. There are customs of announcing the intention in either or both of the places you mention. I prefer it as a petition in the Universal Prayer.
However, the intention is the priest’s intention. It does not have to be announced for it to take place. But the faithful usually want it announced, and I think it’s an easy favor to grant.
